Looking for a Health Insurance Form?

Blue Access for Members® has quite a few benefits, and one of them is the ease of being able to locate health insurance forms when you need them!

Our Form Finder  quickly puts forms at your fingertips. When you need to file a claim or order a refill prescription, nothing is more frustrating than not having access to the forms! That’s why we made these securely stored forms accessible through Blue Access for Members. Easy peasy!

The specific health insurance forms you can access are determined by your plan. If you don’t have dental coverage, then you won’t see dental forms in your member portal.

Here are some forms that you may be looking for:

• Medical, dental* and vision* claim forms
• All HIPAA forms, including the Confidential Communications Request and Standard Authorization Form
• Order forms for both new and refill prescriptions
• Continuation of Coverage

To use Form Finder:

1. When searching, you can search by form name, type or subtype.
2. Use specific words and multiple search terms for better results.
3. Search for complete phrases by enclosing them in quotation marks.

You have the option to sort or narrow your search results:

Sort your search results
You can sort in ascending or descending order by clicking on the column headings for Form Name, Form Type, Form Subtype, Form Number, or Date.

Narrow your search results
The left column (Narrow Your Search) classifies your search results by:

• Form Type - general purpose of each form
• Form Subtype - additional classifications, such as health vs. dental

Narrow down your search by selecting any link in this section. For example, if your search includes Dental forms, clicking on Dental will remove all forms not classified as Dental from your search results.

To see your original search results again, select the View all results link at the bottom of the left column.
